Double vision is medically called Diplopia. A single clear image is something most of us take for granted. Multiple areas of the vision system are responsible for this seemingly automatic process. All of these areas need to work together harmoniously to create clear single vision.

The systems of the vision system are the cornea (the clear outermost disc covering the eye that lets in focused light). The lens (located behind the pupil, focuses light onto the retina). The extraocular eye muscles (necessary to perform the eye’s movements). The nerves (carry visual information from the eyes to the brain). The brain (several areas process visual information received from the eyes).

If any one of these areas isn’t functioning properly, it can lead to double vision. So we have to consider the cause of double vision in relation to the part of the visual system that is not working.

If the problem occurs in the cornea, it often causes double vision in one eye only. The damaged surface distorts incoming light. This can happen because of infection of the cornea or more uncommonly, Lasik surgery can leave one cornea altered causing unequal visual images.

The most common problem of the lens is usually cataracts. Cataracts can obviously distort vision in both eyes.

Muscles Of The Eye And Double Vision

A muscle problem is usually a weak muscle in one eye and that eye can’t move smoothly with the good eye. If you look in the direction controlled by the weak muscle, it causes double vision. Muscles of the eye can be weakened by Myasthenia gravis, an autoimmune illness that blocks the stimulation of muscles by nerves inside the head.

One of the earliest signs is double vision and drooping eyelids. Graves’ disease, a thyroid condition that weakens the muscles of the eye, commonly causes vertical double vision.  That’s where one image is on top of the other.

Several conditions can damage the nerves and lead to double vision. Multiple sclerosis can damage the nerves controlling the eyes.  Sometimes, the first symptoms of Guillain-Barre syndrome, a nerve condition that causes progressive weakness, occurs in the eyes and causes double vision.

Uncontrolled diabetes can also cause nerve damage in one or both eyes, leading to eye weakness and double vision.

Because the nerves controlling the eyes connect directly to the brain, many different causes for double vision originate in the brain.  They include strokes, aneurysms, increased pressure inside the brain from trauma, brain tumors, and migraine headaches.
